I remember seeing the German version when I still had a TV in my flat.. I think the Logo was even the same and it was called "Extrem Schön – Endlich ein neues Leben!", which translates to "Extremely beautiful – a new life at last".
I just checked, it was on from 2009 to 2017 and I`m subscribed to the tv providers streaming service but it seems to have also been deleted.
But I can have a look on some "unofficial" German streaming sites and keep you updated if you like 🙂

I’ve seen polish show based on Swan “chce byc piekna” ( I want to be beautiful), major difference was that everyone went straight to pageant and also contestants were older, mainly in their 40s-50s

Don’t know if anyone has already tipped you: in Holland we have had sort of a similar show, it ran at least from 2016-2018. It was called: Ugly Ducklings (Lelijke eendjes). BUT it is not a pageant. It is a cosmetic surgery show where a team of experts help people who have had big problems with their looks or a bodypart they feel is holding they back. Omg sorry don’t know how to put this down well in english. Also there where Belgian participants as well. I know the Swan was a problematic show but I kinda love it, especially watching with Luxeria and Roly. Some of the transformations really turned out really beautiful and what plastic surgery and cosmetic dentistry can achieve is fascinating to me. I think that if a person truly wants to transform into who they feel they are on the inside I think they should do what makes them happy, and I think it's pretty cool that they can. I know this show did a lot of things wrong and I don't like a lot of the language used but at the same time it's kinda inspirational, it has a anything is possible feeling to it.
